fix: make _IncludedRouter cache rebuild thread-safe Fixes #15974
The lazy cache rebuild in effective_candidates() and
effective_low_priority_routes() was not thread-safe. When multiple
threads triggered the first request concurrently, they could:
1. See a partially-built list (returning 404 for valid routes)
2. Append to the same list simultaneously (duplicating entries)
3. Leak entries into global warnings filters
This fix:
- Builds the list into a local variable before publishing
- Adds a threading.Lock to serialize concurrent rebuilds
- Uses double-checked locking to avoid unnecessary lock contention
- Publishes atomically (list first, version second)
