Add 'input' and 'ctx' fields to the OpenAPI ValidationError schema
to match the actual error response format from Pydantic V2.
Pydantic V2 validation errors include:
- 'input': the actual input value that caused the validation error
- 'ctx': optional context dict with additional error information
This aligns the OpenAPI documentation with the actual API behavior,
improving developer experience when working with FastAPI validation
errors.
